IME Medical Exam vs FCE – Understanding the Key Differences

IME Medical Exam

January 7, 2026 - Independent Medical Examination

For most injury claims, there’s a requirement for medical examination, which significantly differs from a regular medical exam in terms of purpose, process and reporting. These types of examinations are conducted for the purpose of clarifying the actual condition of the claimant and providing medical evidence to process the claim based on facts, not just assumptions, and the claimant’s subjective opinion. Therefore, insurance companies and courts rely on third-party expert opinions to support or counter claims and to provide a clear picture of damage, pain, disability, and the ability to work.

There are two types of medical examinations done for injury claims: Independent Medical Examination (IME) and Functional Capacity Evaluation (FCE). On the surface, these two might seem similar, but they significantly differ in purpose and evidence.

In this blog, we’ll highlight the differences between the IME and FCE, comparing the two.

IME Medical Exam vs FCE Medical Exam

1. Independent Medical Examination (IME)

An IME is a medical exam conducted by a medical expert who isn’t the claimant’s treating doctor or even related to the claimant to maintain impartiality and get fair medical evidence.

Purpose

The purpose of IME is to assess present injuries, determine the connection with the reported evidence and establish future treatment or rehabilitation needs.

Examiner

IME is conducted by medical specialists, including orthopaedic surgeons, psychologists, neurologists, or physiotherapists, depending on the case requirements.

  Selection

IME examiners are usually chosen by insurers, employers, or legal representatives because the assessment is tied to claim decisions.

Timing

An IME medical exam is typically arranged early in the claims process to fully understand diagnosis, causation, and medical status at the time of the claim.

Scope

IMEs typically cover diagnosis, injuries, prognosis, causation, and comparison with the medical history to identify the link between the present condition and pre-existing issues, and to assess the correlation between injuries and the reported incident to support or deny the claims made.

Process

The IME process starts with the doctor reviewing medical records and understanding the injury background. During the appointment, they ask questions about symptoms and perform a focused physical exam to check movement, strength, and pain. The examiner then prepares an independent medical opinion based on all findings.

2. Functional Capacity Evaluation (FCE)

An FCE is also a medical exam conducted to obtain an objective opinion, but it differs in purpose and process from an IME.

Purpose

The First thing that differentiates FCE from IME medical exam is purpose. Unlike IME, FCE determines functional abilities, movement limitations, and working capacity.

Conducted By

Physical and occupational therapists, or rehabilitation experts, usually conduct FCEs.

Examiner Selected By

FCE evaluators may be selected by rehabilitation providers, employers, or insurers, depending on the case.

Timing

An FCE is usually completed later, at the post-recovery stage, to evaluate the claimant’s functional abilities and limitations after complete recovery and medical stability.

Scope

FCE is more related to the physical capacity, endurance, range of motion, lifting ability, job-specific tasks, and overall functional limitations. This in-depth analysis measures an individual’s ability to return to work and perform work-related tasks.

Process of Evaluation

The evaluation process of FCE also differs from that of IME. In contrast to IME, FCE is more related to physical tests, repetitive motion assessments, strength measurement and pain-level assessment to measure one’s functional capacity using standardised criteria for everyone.

In conclusion, while an IME provides an objective opinion on the claimant’s condition, clarifying the real cause, extent of injuries, and likely recovery timeline, an FCE focuses on functional capacity after recovery, showing readiness to return to work, any limitations, and required adaptations. In legal claims, both the IME and FCE play crucial roles, each fulfilling a different but equally important need. Whether you are creating an IME report or an FCE report, both are equally daunting tasks that require a lot of attention to detail, editing, formatting, and meticulous checks to ensure accuracy, consistency in language and evidence, and compliance needs. All of this takes hours of work, especially with outdated software or manual processes. But not anymore!
